The Hamden Transition Academy is a multi-faceted, community-based program serving students with intellectual and other disabilities.

Hamden Transition Academy is a community-based collaborative program between Hamden Public Schools and Southern Connecticut State University, which helps young adults with disabilities transition from school to independent adult life.
